The Employer has advised that the one percent payment retroactive from October 1, 2021, and the 1.25% payment retroactive from September 1, 2022 will be included in your November 2, 2022 pay.
The one percent Covid lump sum (one percent of the Basic Rate of Pay for all hours actually worked between January 1, 2021 and December 31, 2021) will be paid November 16, 2022.
Former employees are eligible for retroactive payments, which they must request by December 5, 2022. While Article 1.04 of your collective agreement states that such request must be received within sixty (60) calendar days after the ratification of the agreement (September 29, 2022), both parties have agreed to extend this deadline.

- Employee # (if possible)
- Union/Local
- Current address & phone number (to facilitate payment)
