Please fill out and submit this form if you require accommodation for a Local 040 event or meeting.

Your request form will be sent off to members of the Local 040 Executive team for processing and booking as needed.

Double-occupancy/shared bookings are completely covered by Local 040; however, we do offer the option for members to pay the difference in price if they would like their own room.

DISCLAIMER

If I fail to notify the Local that I will not be attending and the room cannot be cancelled, I may be responsible to pay the cost of the room.

Should I not book my room at least 2 weeks before the event through the Local, I will be responsible to find and book my own room at my own cost to be reimbursed by the Local.

Each incident will be looked at individually by the Local Executive and a decision made before reimbursement will occur.
